The CD-R promo came in a clear slimline case, and was handed out at Gamers Day in San Francisco, California on 2003.01.16.
CD-Rs are commonly used for promotion in the music industry if small numbers of discs are made, usually up to 100 or 200 copies. It's cheaper and quicker for small quantities, as they do not have to manufacture pressing plates. The insert is of high paper quality, and can be opened to reveal an image of the church and lyrics to the two vocal tracks.
The disc is very similar to the European CD-R version. The difference is that the URL on the disc and insert is changed from konami.com to konami-europe.com. There is also a European pressed CD promo which came in a different plastic case, resulting in a slightly different insert.
