Polish Joel Edition. It came in a cardboard box holding a canvas wrap with multiple pockets for all the items. A cut-out in the box partially shows the front art of the canvas wrap. Contents:

The box seems to be labeled BCES 01584/J, but the disc is BCES-01585.
