The playable demo of Bio Hazard came with a triangular warning sticker on the front and a survey/ registration card. The insert can be unfolded to reveal the controls and other information.

Among other things, the demo contains the unfinished puzzle with blood on the wall on the second floor which was taken out of the final game.
