The box collects Biohazard and Biohazard II Apocalypse in a special packaging. The discs are stored in a three-panel digipak housed in a slipcover. The digipak also has a pouch to store an index card for the first film, two advertisements for Biohazard 4 and related merchandise, a film catalog, and one more paper item. An obi is wrapped around the slipcover.
